I really enjoyed Heinz Bloch's article, "Looking for RCFA databases" (Jan. 2002). He touched on several very important points. I think one problem with the article may relate to terminology usage. Mean time between failure (MTBF) should be redesignated, mean time between stoppages (MTBS). The availability calculation must consider stoppages for routine maintenance. Considering real-world machine installations, one of the weakest links in the reliability chain is lube oil. When using conventional mineral oils, the maximum MTBS is going to be 364 days.
