Some process safety incidents occur as the result of simultaneous or sequential occurrences of operations or activities that interfere or clash with each other. These interactions increase the risks of the activities, or create new risks, resulting in undesired events or circumstances that can have an adverse impact on safety, the environment, property, etc. Simultaneous operations (SIMOPs) and sequential operations often involve work in the same area by multiple contractors and subcontractors, or by multidisciplinary workers, whose work may overlap and/or interact. They sometimes occur when non-routine work is performed, including safe work and the commissioning and startup of processes. S
