In May and June 2000, three incidents took place at BP's Grangemouth, Scotland, UK, complex. A new report details results of the investigation into the events. What happened over a few weeks? There was: 1) a power distribution failure, causing refinery shutdown, 2) failure of a steam main resulting in minor injury to a member of the public and 3) a serious fire at the FCC unit. Although worst-case scenarios did not play out, each incident had the potential to cause fatal injury and long-term environmental impact. The wide-ranging inquiry extended beyond the plant and installation to examine human factors, health and safety
